DENTAL INSURANCE INFORMATION
As a convenience and courtesy to our patients, our office will submit charges for services to your insurance
if you carry dental insurance. However YOU, the patient is ultimately responsible for account balances.
In other words, the service provided by our office is an agreement between you, the patient, and our office.
The insurance relationship is an agreement between you, the patient, and your insurance carrier.
WE STRONGLY RECOMMEND THAT IF YOU'RE UNSURE WHAT SERVICES YOUR INSURANCE COVERS,
YOU CONTACT THEM DIRECTLY.
We will do our best to inform you or help you understand your coverage, but with over 6,000 different insurance
providers and policies, ultimately the last word lies with your carrier. Most insurance plans are not designed to pay the
entire fee for services provided, and therefore you will be required to pay any difference between benefit
payments and the actual billed amount. Most policies have deductibles and/or co-payment clauses as well.
Also, your insurance carrier may pay benefits directly to you, the patient. If this is the case you must ask your
insurance company to assign the benefits to our office by signing the appropriate agreement section of the insurance submittal
forms. Call your insurance carrier directly if you have further questions in this area.
